Records the decisions from the planning session: three formats with ADF as hub, own-output losslessness for markdown and HTML, opaque carry for unknown nodes, full CommonMark input with canonical fixpoint, directive syntax, formats-are-semver, publish-on-version-change with Renovate automerge, personas instead of named consumers, and the reordered milestones (0.1.0 = markdown round-trip).
