Behaviour is now explicit toggles: `CACHE_TEMPLATES`, `REQUIRE_SECURE_SECRETS` (parsed/validated in `config.ts`, wired via `server.ts`); compose files set them per deployment. `app.ts` no longer reads `process.env`.
