The maintainer's answer on 2d's open question: a markdown serializer spells a node by type without checking its position, and ADF keeps such content in an unsupportedBlock rather than dropping it, so the three shape refusals become carries. AGENTS.md 3 and spec/flavour.md lose the exception; todo.md 1d loses the blocked note.
