Plan 3's first chunk, no behaviour change. defs/ becomes codec/; files move into protocol/, messages/ and session/, each named for what it exports; options.ts holds the one defaults table; bind directions move to protocol/bind.ts; result.ts absorbs error-from.ts. Retires the "src/ stays flat" decision for "src/ is grouped by layer". Closes the three defaults, the session-options.ts split and the flat src/ items.
